Description
he overarching aim of this service is to support people to live independently in their own homes as long as possible. This will be achieved through the following objectives: ⢠Deliver Wirral Independence Service including the Telecare Monitoring and Response service and Falls Prevention Service ⢠Provision of a Community Equipment Service (CES) for adults and children within the geographical boundary of Wirral Borough Council including those who live out of Borough and are registered with a Wirral GP. The service includes. ⢠Pressure care, ⢠Continuing Health Care (CHC), ⢠Adult Visual Impairment & Hearing Impairment equipment, ⢠Telecare and any future innovations (equipment, installation, maintenance, monitoring, decontamination and repair and recycling). ⢠Mobility aids, ⢠Moving and Handling, ⢠Bariatric, ⢠Schools Specialist Equipment, ⢠Paediatrics, ⢠Nebulizers ⢠Toileting ⢠Specialist non catalogue items ⢠Minor Adaptations ⢠Carers Emergency Response Cards Incorporating: ⢠an online IT requisition. ⢠monitoring performance dashboard and reporting system, ⢠Community based Retail Outlet/ Demonstration facility (Centre for independent living) ⢠online self-help/assessment system and to include purchase and recycling opportunity ⢠suitably qualified, knowledgeable, and experienced staff to support those funding their own care and to provide professional assessment and oversight within the service
