Description
Nottingham City CCG invites professional qualified suitably experienced providers to evaluate the effectiveness of the Health & Social Care Transfer to Assess & Care Home Vanguard Beds Pilot models.
The successful provider will be required to determine
⢠Whether and how the pilots have achieved their objectives? ⢠What impact the pilots have on the lives of the individuals receiving the services? ⢠What works well and what doesnât work so well and why? This should consider the clinical models and input from different professions. ⢠Whether the pilots have improved health and social care outcomes (measurable and actionable) in the short term? ⢠Whether they have reduced costs and improved system efficiencies? ⢠Whether/how the model supports the recommendations within the new NICE clinical guidance (NG27) âTransition between inpatient hospital settings and community or care home settings for adults with social care needsâ.
