Our joint ambition is to improve health outcomes for people in Merton. As part of this, we want to strengthen the role of community services in improving the health of the community and thereby reduce the need for acute care.
We want provision that is based on an integrated approach, with community services playing a leading role in enabling effective working between different professionals and organisations, so ensuring that people in Merton experience care that is seamless and well co-ordinated.
We know that there will be many changes over the years ahead. Hence, we are seeking a Provider who will work flexibly, responsively, creatively and in partnership with commissioners and other partners; who will foster a âcan doâ attitude amongst staff and embed a culture of continual learning and development across the organisation.
Putting peopleâs needs first and delivering improved outcomes for them must be at the core of all community service activities at all times.
Additional information: Our vision is for community services that provide the right care, at the right time, in the right place achieving the right outcome for each individual person.
We aim to ensure that: ⢠People are placed at the heart of their care, and the care takes account of their individual needs and preferences. ⢠People receive high quality care, clear communications and information and are treated with compassion by skilled and motivated staff. ⢠People are helped to improve their health and wellbeing and prevent ill health in themselves and those they may care for. This is particularly important to those with long term conditions. ⢠People receive integrated and seamless care. ⢠People receive care in the most appropriate setting closer to home where possible.
