Description
NHS Cumbria Clinical Commissioning Group (CCG), wish to commission a service for the provision of a Falls Prevention Service in the Eden Locality. This Request for Quote (RFQ) will allow bidders to bid for the 11 month pilot term. Please note that commissioners wish for this service to commence 1 May 2015.
The aim of the service is to provide an evidenced based, patient centred, high quality, value for money, service for the prompt treatment and support of patients who have yet to fall or are at risk of falling within a community setting. The service must be delivered where there is identified need at the convenience of the patient in their community where it will be easily accessible. The North of England Commissioning Support Unit (NECS) is managing the procurement process for and on behalf of NHS Cumbria CCG. NECS is utilising an electronic tendering tool to manage this procurement and communicate with potential providers. Accordingly, all communications and submissions will be conducted via the In-Tend Portal, https://in-tendhost.co.uk/necs. It is free to register on the portal, which can be accessed at any time of day. A dedicated helpdesk is available should potential providers have any queries about, or problems with, the portal; 0844 2728810 (Monday to Friday 8am to 5pm) / [email protected]. The financial threshold for the service is £64,166 for the 11 month duration. Bidders are requested to complete a Financial Model Template (FMT) indicating their costs for running this service. Bidders will be scored on how far below the threshold they can deliver the service for as detailed within Document 1 - Request for Quotation Document. Bids received exceeding the financial threshold will be deemed non-compliant and will take no further part in the procurement. The RFQ Information Document and supporting documents are available to view / download within the In-Tend Portal. Please note the deadline for submission is 12 noon 09 April 2015.
