Description
NHS Leeds West CCG is looking to commission an organisation to provide a social prescribing service, signposting services between GPs and Voluntary Sector Organisations. The service will be provided across all 37 practices within NHS Leeds West Clinical Commissioning Group (LWCCG) and will include self-referral.
