Healthcare services cannot operate in isolation and must be delivered in line with key principles of integration, including the implementation of three ICNs across the borough to deliver better integrated working between primary, community, and all other health and social care services. By integrating care pathways, patients will have better access to the right care, closer to home, from the most appropriately skilled professional and at the time of need. Patients will be at the centre of care planning, commissioning, delivery & support, resulting in a greater focus on ill-health prevention, delivering proactive & responsive care when it is needed and helping to maintain patientsâ independence for as long as possible.
