Description

Discharge to Assess (D2A) is designed to reduce unnecessarily prolonged acute hospital stay. It is a multi-agency approach between the Authority and SWFT including Continuing Health Care, which allows assessment for ongoing services to take place after discharge from hospital.

There are three pathways within D2A:

Pathway 1: For customers who can return home with additional support from CERT or Reablement. Pathway 2: For customers who cannot be discharged directly home but who have the potential to do so if given additional rehabilitation. Pathway 3: This is for customers who are likely to need a care or nursing home placement in the longer term

The scope of this tender is the commissioning of residential home places (beds) from which Pathway 2 services can be delivered.
