Description

Prior Information Notice.

Ealing Clinical Commissioning Group is currently piloting a Primary Care Patient Transport service for patients who face barriers to accessing services in primary care settings because of the lack of appropriate transport and associated support.

The pilot service is specifically aimed at supporting patients who are elderly/frail or children who have mobility issues or other health problems which restricts their ability to access public transport. The service is for patients who are registered with GPs across the borough of Ealing

Ealing Clinical Commissioning Group now wish to give prior notification of their intention to re-commission the current Primary Care Patient Transport service and wishes to ascertain market interest in this continued provision.

Invitation to Prior Information Notice Event 1100hrs Thursday 28th July 2016

The Commissioner would like to invite interested potential providers to attend a Prior Information Event on Thursday 28th July 2016 between 1100 and 1300 at:

NHS Ealing Clinical Commissioning Group 3rd Floor Perceval House 14/16 Uxbridge Road Ealing W5 2HL

Interested parties are requested to register for this event by 12:00 pm by Tuesday 26th July 2016, by emailing the Ealing CCG Primary Care Patient Transport team, on [email protected]

The Event will provide opportunities to: — Understand the current service provision — Understand the CCG’s commissioning intentions for the re-commissioned service from January 2017 — Meet and network with peers and/or potential partners.

To ensure that the future Primary Care Patient Transport service provides the best all round outcomes for service users, the CCG is interested to consider the views and advice of those with experience in the delivery of specialist Primary Care Patient Transport Services.

General The CCG has identified a range of patient needs in relation to transport to appointments in primary care. In particular patients who: experience difficulty attending appointments due to lack of transport; are elderly or frail; have mobility or other health problems; and live in geographically isolated parts of the borough.

Having mapped potential demand for this service and established a simple eligibility criteria, the aim is to enable patients to access primary care more easily, encouraging attendance, reducing the need for home visits and reducing the level of activity at secondary care acute providers, in line with the CCG’s Out of Hospital Delivery Strategy.

Potential Contract Cost The estimated annual value for the Primary Care Patient Transport Service will be around £290,000 to £310,000. It is envisaged that the CCG will be looking to enter into a three-year contract, commencing January 2017.

Additional information: Service Eligibility Criteria 1.1 Patients and their carers who are registered with GPs in Ealing and meet the agreed eligibility criteria for the Transport Service, to travel to primary care locations for appointments. 1.2 Eligibility criteria for the service are based on whether the patient can be transported without risk to their safety or the safety of others, and whether there is a clinical need for transport based on disability, a lack of mobility, or other health problems. 1.3 The Transport Provider shall be responsible for the safe, timely and comfortable transport of patients between their place of residence and the healthcare facility in question and back again.

Key outcomes/objectives will include: • Reduced isolation and increased social integration with others • Supported ways to deal with loneliness and support social interaction • Reduced ‘Did not attend’ (DNAs) in primary care • Increased follow up care for patients • Proactive primary care with the aim of improving health outcomes • Reduced mortality rates • Reduced hospital admissions • Improve outcomes for patients through the provision of Primary Care Transport • Provide care closer to home in line with the Out of Hospital and Whole Systems Integrated Care models.

Social Value This service will contribute to the social, health and well-being needs and challenges facing vulnerable elderly patients in respect of loneliness and isolation: • Loneliness and isolation can often lead to mental health problems and physical health problems • Improving opportunities for the vulnerable to socialise and integrate with others. • A resource to make an impact on a patient’s health and well-being removing barriers to enable access to healthcare services.

Older people often require additional support to navigate leaving their homes and accessing health services. It is these hard to reach people who often have the greatest health needs and would benefit the most from services such as patient transport.

Additional information If you are unable to attend the event but would like a copy of the presentation materials used at the event, please contact [email protected]. In all cases, the CCG alone will retain responsibility for deciding on the contents of all procurement documents.

Providers should note that this is a Market Information/Assessment Exercise and that no procurement may follow this process.

The final contract value, procurement approach and contract structure for these services are currently being finalised. Any expenditure, work or effort undertaken by organisations or individuals prior to any subsequent procurement for this service is a matter solely for their own commercial judgment. The CCG reserves the right to withdraw from the process at any time or to not tender these services or to tender on a different basis.

It is envisaged that if a decision is subsequently taken to proceed, tendering is likely to begin in mid-August 2016
