Description
The Council wish to commission a new NHS Health Check Service: that utilises both Primary Care and the community infrastructure with the aim of improving the health and wellbeing of eligible adults; through the promotion of early awareness, assessment and management of the major risk factors for Cardiovascular Disease (CVD). A key focus is to prioritise and engage with those who are vulnerable and the most at risk, including those who have been the most affected by COVID-19 and those that engage less well with Primary Care, to contribute to the aim of reducing health inequalities within Doncaster.
Providers will be required to demonstrate flexible and innovative solutions for active engagement and increasing participation of Service Users, local communities and target populations and demonstrate how this will be used to inform the development of Services.
