Description
On behalf of the two CCGs in Northern Staffordshire , population circa 514,000 , North Staffordshire and Stoke on Trent CCGs wish to commission a Community Falls Service for the North Staffordshire and Stoke-on-Trent CCG population. To minimise the harm due to falls and the number of associated hospital admissions there is a requirement to focus on prevention both primary and secondary.
