Description
NHS Calderdale Clinical Commissioning Group (CCG) as the Contracting Authority is seeking to identify a suitable provider to provide primary care medical services for the practice populations currently served by the following practices when the current Alternative Personal Medical Services (APMS) contracts expire at the end on March 2019:
Meadowdale Group Practice:
Currently providing services from Rosemount House in Elland, Nursery Lane in Ovenden and Allen House in Sowerby Bridge with a registered list of approximately 3,600 patients
Park and Calder Practice:
Currently providing services from Horne Street in Halifax (Park) and Todmorden (Calder) with a registered list of approximately 5,200 patients
Under an APMS contract the successful Bidder(s) will be required to provide high quality primary medical services to the practice populations referred to above and ensure continuity of service provision for patients.
The CCG has not yet formed a view whether the provision of services to the above practice populations will replicate the current configuration of provision and is keen to receive and explore options in line with the strategic intent for primary care working at scale and close integration of services within localities. The CCG will review the response to this Prior Information Notice to refine intentions in respect of the scope of service and whether the contract will be offered as one or a number of lots.
The CCG is keen to receive expressions of interest from suitably qualified potential bidders who are either individual organisations or coming together as a group of organisations.
Your response should include:
⢠Overview of your organisation(s) and experience in providing primary care medical services ⢠Whether your interest is in part and therefore specify which, or all of the opportunity
