Care Navigator service supports people from hospital settings with short term information and support to help them settle back into the community. This service is vital to reducing readmissions to hospital and ensuring adequate support is provided. The service is trusted by all partners and offers effective challenge to both hospitals and social care providers. The independent nature of the role is welcomed by all parties and is seen as vital to encouraging positive relationships with primary health.
